Won't play through the headphones is strange indeed. I envision an office
full of people with MM PCs and speakers on their desks and a cacaphony of
sound where no one can hear anything. Sounds like a design flaw to me.

But sound cards may have a headphone jack (as may the amplifier that they
are plugged in to).

Why buy a sound card for every PC in a room full of people when all they
need is a headphone anyways? and there is one built into the CD-ROM? Most
companies I deal with are trying to save money right now.

Yes, but the sound from the CD-ROM headphone will never be well coordinated
with the action on the PC. (Hence the need for CD-ROM XA).
Besides, sound cards can be had for under $100. And the Disney Sound
source is under $40.

I'm missing the context of your question. modern cd rom drives incorporate
the headphone electronics right on the disk drive's controller board (the
drive, not a PC add-in "drive controller board") and are only for "CD
audio" sound (ie, you can play a CD record in a CD ROM drive; it's very
easy).
For the Toshiba drives there's a very simple modification which turns it
into an automatic CD Audio player (even unconnected to a computer). See my
