I have tried both "Hitachi" and the generic option.

Intel Premiere/PCI II

If I understand your question correctly (ie if PCI BIOS = motherboard BIOS) it
is
AMIBIOS 1.00.05.AF1

VIP.EXE reports ROM Version 3.12. (Is this what you are looking for?)

I have a similar problem with NT 3.5. I have a Batman board with bios
1.00.06.af1, Viper PCI bios verions 3.10 and an Adaptec 2940.
I have taken the advice of Diamond and upgraded and still NT does not work. I
believe Microsoft are working on a fix. What I like to know is what is at
fault, NT, Viper, motherboard or just the combination?

Please try using a lower refresh rate, and possibly a lower resolution. Also,
what are the specifications for the monitor?
