Sounds like you need a database with a forms designer. That
would either be superbase personal 4 or superbase pro 4. There is a
learning curve that goes with these things, so you might want to look
at some of those packages like "Invoice It". Maybe it could be
adapted to that purpose. BTW, what's your setup.
