Donald,
Another possibility is that your object transform is skewed from the world
coordinates. Say, like if something came in from the lofter sideways and you
had to rotate it to get it right-side-up,
and hadn't done anything else to cause it to fix itself.
Also, if the path you're referring to is an imported spline path, you've got to
be real careful of the vertex/adjust arrows.
And a yet another thing it might be is if you've already applied rotate keys to
this object, whether manually placed or acquired from a previous attempt at
paths/follow, you've pretty much got to delete 'em, then if you need more
rotation anywhere, put it in after your paths/follow is the way you want it.
