I don't see anything blatantly suspicious, but the mouse driver may be funky. Try using a Microsoft driver, or logitech. Also, I don't think this has anything to do with the problem (probably not…), but try it anyway: Smartdrv has cause problems on my system in the past (nothing like what you describe).
