John, I've got a rebate form right here. It requires you to send:
a: The serial number bar code from the DeskJet box.
b: A certain page from the instruction manual.
c: A copy of the sales invoice.
d: An old printer, which will be "destroyed" upon receipt.

They also give a phone number – 1-800-553-7553. If I were you I'd call
that number to ask about getting the form. If you didn't buy the printer
from an "authorized HP Dealer", you might have a problem getting the
rebate. Finally, I hear that there is also an alternate program to get a
free font cartridge instead of the rebate. I assume that deal is also run
by the same marketing company. I'm trying to get my dealer to find out
about that before I send in the stuff.
