yep, it's the KA-1 Series II keyboard adaptor by Top Secret Device
(606.276.3779), lists in AC Guide for $54.95 and it was also mentioned in
AmigaUser. only downside, it requires an XT keyboard for some reason. i am
using it with a Keytronics keyboard that has an xt/at switch.
