I had a problem like this last year, and it turned out to be the hard disk
controller card (el cheapo at $30). As soon as I replaced it with a more
mainstream one all problems ceased. I have no idea if this is the problem with
yours, but it's pretty cheap to replace the controller for a (reliable) name
brand so it may be worth a try.
