You need to talk both to your ISP and to the phone company. The ISP has to tell you where the other end of the leased line will be connected, the phone company will tell you what the costs will be to connect that leased line. Expect line costs to be in the hundreds per month, minimum, plus whatever the ISP is going to charge for Internet connection services. No, you can't use standard copper for better than 56k (and I don't think they use it for 56k except with ISDN). They don't run a new line all the way between your site and the ISP POP, but they do run a new line from the local office to your site.
