The easiest thing is to just use a standard MAC compatible 25 to 50 pin SCSI cable. On your SCSI drive, you can then just use a 50 pin SCSI connector to a standard 50 pin header connector. That's much easier than trying to make up a special cable.

It sounds like you are having IRQ problems. Try changing the jumper position at DACK2 to another DACK position on the jumper block and see if that helps at all. If that doesn't work, then your motherboard may just be incompatible with the LL upgrade method.
You can try forcing an IRQ if you like… make sure you add the /Ix command line parameter for the IRQ number as well as the jumper on the T130. The address of the card shouldn't make a difference, but you can try changing that, too, if you like. That's…

Sounds to me like you may have a cabling problem. Best bet is to contact the Adaptec Tech Support line at 800-959-SCSI (800-959-7274) I'm not really up to speed on the 1540 series products, but they can give you the right info there.
No, as far as I know, they haven't been recalled. There have been some in-line changes made because of problems found with some poorly shielded cables on the market… but I wouldn't worry about your existing cards and setups at all.
I haven't found termination to be a big issue with the PAS and multiple devices. You DO have to make sure you are using different SCSI id's for each device and you DO have to install both the HD and CD drivers from the SCSIworks! package… you can't use the…
With DOS 6, the only thing that is different is you need to use the MSCDEX.EXE that comes with DOS 6. Earlier versions are not compatible. Other than that, there shouldn't be any problem.
